VIEWER DISCRETION ADVISED: This video is very graphic. An open thoracotomy is performed on a motorcycle accident victim who was initially talking at the scene and became unresponsive and lost his vital signs in the ambulance minutes away from the hospital. A heroic attempt was made to save his life. Unfortunately, despite return of spontaneous circulation the patient died in the operating room.

The Committee on Trauma of the American College of Surgeons takes the viewer into “The Management of Gunshot Wounds of the Abdomen.” Made under the supervision of Dr. Robert J. Freeark, a surgeon who established the nation’s first trauma unit, the film includes graphic scenes of a gunshot victim’s wounds and of abdominal surgery. It opens at mark 00:54 with the arrival of the victim at an emergency room. Such an event, we are told, sets into motion a standardized management plan, which is shown on the screen: resuscitation, tracking the missile, preparation for surgery, and operative repair. The film then details each step, beginning with resuscitation at mark 01:25, as ER personnel take vital signs, draw blood, and conduct a physical exam — including a rectal exam — which is shown at mark 03:21. At mark 03:48, the film delves into the tracking of a missile, including inspection of the wound on a victim and x-rays. Mark 04:51 shows a doctor inserting a tube into a patient’s side at blood and pus drain out to prevent a potential pneumothorax. The patient is ready to be prepared for surgery by mark 07:23, including an examination of the peritoneal cavity, as a physician probes a victim’s buttock, hips, and groin after he was shot in the side. Once the gunshot victim...

In this first episode, the team get straight to work. If they are to save lives, every second is vital for the trauma doctors in their search for clues. For a motor cyclist whose foot is mangled in a car accident, the team have to check every part of his body and the wreckage for clues to other injuries so that they can make the correct diagnosis.

London's Air Ambulance doctors race to stabilize a 19-year-old woman after she is hit by a van, and the team's investigation reveals a potential spine injury with grave consequences.

Weak, nauseous and pale, an elderly woman's confusion makes the hunt for clues to her illness difficult. After a series of tests, the team finally make an unexpected diagnosis. After a 45-year-old cyclist with a head injury is rushed in by helicopter, the patient's memory loss provides the team with a vital clue to his condition.

The skull is an amazing structure; surgeons repair a man’s face after severe trauma. This video will assist in determining who you can successfully rescue in an emergency situation. At first sight a victim may accidentally be presumed dead. However evidence such as this video suggests the if the skull is intact the human body can recover not to mention other major components.

Last year, there were more than 50,000 shootings in the United States, and more than 25 percent of those shootings were fatal. In Philadelphia, where gun violence has reached a three year high, two researchers are launching a city-wide study that they think can save the lives of more shooting victims.

When a shooting victim is rushed to the hospital by ambulance, they are usually treated with what’s called advanced life support, which includes IVs, breathing tubes and defibrillators.

But two trauma surgeons at Temple University Hospital in Philadelphia believe that if Paramedics and EMTs switched to only Basic Life Support, which involves less invasive techniques, more lives would be saved.

Studying gunshot victims in Philadelphia, means studying mostly young, black men. And the medical profession has a dark history of using black people as the subjects of dangerous experiments. To avoid any suspicion, Temple University has engaged community leaders in their study.

Antonia Hylton went to Philadelphia to find out more.
